Nurse Care Manager – Division of Geriatrics & Palliative Medicine

Karolyn is a Registered Nurse with her Associates in Nursing from the Community College of Rhode Island.

Meet your Nurse Care Manger! Karolyn is available to all patients and families to help you with any of the below services, and more:

• Provide follow-up on any recent visit to the hospital
• Provide education and learning materials about your health and offer knowledge to help you manage your health
• Help you manage your medications
• Work with you to develop a care plan for you in alignment with your primary care physician.
• Teach group education and provide community resources for you

Your Nurse Care Manager will work directly with you to help you achieve your health goals and these services are of no cost to Brown Medicine patients!
