Aiming to Increase Care Coordination and Support for Caregivers
Brown Medicine – Geriatrics proudly presents GUIDE, a Centers for Medicare and Medicaid Innovation Model to support patients living with dementia with and without caregiver. GUIDE provides comprehensive assessment, longitudinal care navigation and caregiver support. After the assessment, our GUIDE team provides a written dementia care plan to patients, their caregivers, and their primary care providers. We offer care navigation and help anticipate and address patient needs with dementia-related concerns, including caregiver stress, education, and caregiver training and support in dementia care. Our interdisciplinary team of Geriatricians, Psychiatrist, Social worker, Nurse Care manager and Care Navigator stand ready to help.
Patients and caregivers can contact us with any dementia-related care concerns. We can also help with access to clinical and non-clinical services such as meals, mobility and transportation. GUIDE program can arrange up to $2500 per year of relief care for caregivers who meet the CMS guidelines.
To be eligible for GUIDE the patient must have both:
- A diagnosis of dementia which includes unspecified Dementia, Major Neurocognitive Disorder, Alzheimer’s Disease, Vascular Dementia, Lewy Body Dementia, Frontotemporal Dementia, or Mixed Dementia
- Medicare Part A and B enrollment
- Medicare Advantage plan enrollment disqualifies participation
Brown Medicine – Geriatrics offers GUIDE in collaboration with the patients’ primary care and specialist providers, who will continue to provide their regular care.
